Scheduling a First Appointment

A referral from your physician or insurance company is usually needed when you schedule your first appointment.  Some information you will be asked is your name, home telephone number, referring physician and your insurance company.  Among the information you will be given is that your co-payment is due weekly and that if you are unable to come for your appointment to please call in advance to reschedule.
